Housing Case Manager– Sudbury & Sudbury East

  • Department: Community Development and Housing
  • Employment Status : Full-time, Permanent
  • Hours per Week: 35 hours per week. May include weekday, weekends and evenings
  • Work Location : 111 Elm Street, Sudbury, Ontario. Position travels 3 days per week to work independently in community housing buildings located in Warren, St. Charles and Noelville. Mileage is paid from Sudbury
  • Union Membership  : This position is a unionized position (OPSEU)

RESPONSIBILITIES:
Reporting to the Coordinator, Housing Case Management and working in partnership with the Manitoulin-Sudbury District Service Board (MSDSB), the Housing Case Manager provides supports to individuals residing in community housing units in Sudbury East, as well as individuals residing in Sudbury. Other duties include:

  • Build rapport and assist with tenancy-related issues
  • Provide support to individuals with the development of goal-focused care plans and wellness strategies
  • Provide direct, hands-on support to individuals in maintaining their living environment and assistance with instrumental activities of daily living skills
  • Develop educational programs relevant to identified needs
  • Ensure all file management and reporting functions are maintained in a comprehensive and timely manner

QUALIFICATIONS:

  • Completion of a 2-year diploma or degree in a health or social services discipline.
  • Combination of post-secondary education and 5+ years of experience that reflects the job description and requirements of the current position will also be considered.
  • Minimum one year experience in related position.
  • Access to a reliable vehicle required to complete job duties
  • Proficiency with Microsoft Office software required
  • Knowledgeable of community agencies and resources
  • Bilingual requirements will be posted in accordance with the French Language Services (FLS) Designation Plan. Bilingualism is required for 40% of positions. If a position is designated bilingual and non bilingual candidates (both internal and external) meet the qualifications, unilingual candidates may be considered.
  • Professional Designation/Certification (one of the following required)

a.     Registration, in good standing, with a regulating professional body Ontario College of Social Work and Social Service Worker (e.g.OCSWSSW); or

b.     Certification, in good standing, with a professional certifying organization* within health or social service fields e.g. Certified Psychosocial Rehabilitation and Recovery Practitioner (CPRRP) from Psychosocial Rehabilitation (PSR Canada); or

c.      Working towards obtaining one of the above within 6 months of hire

*Organization must have a written code of ethics, core values and standards that are consistent with those of  CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M.

SALARY INFORMATION:
Bachelor Degree

·        Hourly, Step 1: $27.57 – Step 4: $32.44

College Diploma

·        Hourly, Step 1: $24.87 – Step 4: $29.26

*Successful candidates for this position will start at Step 1 of the salary grid.
