RESPONSIBILITIES:

Under the direction of the Coordinator, Housing, the Case Manager, Residential provides person-centred supports to individuals within CMHA-S/M residential settings. CMHA-S/M operates several residential programs that provide support to individuals living with mental health and/or substance use concerns. Working collaboratively within an interdisciplinary team, the Case Manager integrates case management support with the development and facilitation of structured recreational and wellness-based activities to enhance residents’ quality of life, promote recovery, and support independence. Responsibilities include developing, updating and implementing wellness plans and programming that align with residents’ wellness goals.

QUALIFICATIONS:

  • Graduation from a 2-year College Diploma Program or completion of a University Degree in a Human Services area.
  • Minimum 1 – 2 years’ experience working with individuals, families in the mental health system.
  • Computer skills: Microsoft Word, Excel, PowerPoint, and Outlook.
  • Experience with Individual Record Management and Assessment Software.
  • Valid Ontario Driver’s License.
  • Access to a reliable vehicle.
  • Automobile liability insurance coverage in the amount of at least $1,000,000.
  • Knowledge of applicable legislation, community agencies and resources.
  • Excellent interpersonal and communication skills.
  • Fluency in English is essential. Advanced conversational French is essential for 40% of these positions. Priority will be given to candidates who are bilingual in French and English as per our French Language Service Plan. If a position is designated essential bilingual and no bilingual candidates (both internal and external) meet the qualifications, unilingual candidates may be considered.
  • Professional Designation/Certification (one of the following are required):

1.     Registration, in good standing, with a regulating professional body Ontario College of Social Work and Social Service Worker (e.g.OCSWSSW); or

2.     Certification, in good standing, with a professional certifying organization* within health or social service fields e.g. Certified Psychosocial Rehabilitation and Recovery Practitioner (CPRRP) from Psychosocial Rehabilitation (PSR Canada);  or

3.     Working towards obtaining one of the above within 6 months of hire

*Organization must have a written code of ethics, core values and standards that are consistent with those of CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M.

SALARY:

  •  Bachelor Degree: Hourly, Step 1: $28.12 – Step 4: $33.09
  • College Diploma: Hourly, Step 1: $25.37 – Step 4: $29.84

*Successful candidates for this position will start at Step 1 of the salary grid.
