UPDATE September 25, 2023:

Ontario’s school and child care specific COVID-19 online self-assessment tool has been replaced by the general self-assessment tool. Individuals can take this NEW Screening Tool  if they have symptoms of illness or test positive for COVID-19.
It is important to carefully read and follow the instructions provided with the screening result. Anyone who is feeling sick or has symptoms of illness should stay home and seek assessment from their health care provider if needed.

Copies of this new Screening Tool as well as 5-pack Rapid Antigen Test Kits (for home use) are available upon request at all of our locations.  We will also have masks and hand sanitizer available, should you choose to use them.
