Case Manager, Healing with Hope

  • Department                     : Harm Reduction and Shelter Services
  • Employment Status        : 12-month contract with a possibility of extension
  • Hours per Week              : 35 hours per week
  • Work Location                : 200 Larch Street. Sudbury, ON

RESPONSIBILITIES:
Under the direction of the Manger, Harm Reduction Home, the Case Manager provides rehabilitation and comprehensive personal support to individuals experiencing mental illness/substance users. Duties of the Case Manager include:

    • Provide professional support services within the framework of an individual directed recovery and rehabilitation model.
    • Work within an interdisciplinary team and provide comprehensive case management support involving community treatment, psycho-social rehabilitation and assessments, goal setting, development of care plans and support to the residents of HWH.

The Case Manger will be required to work weekdays, evenings and weekends.

QUALIFICATIONS:

  • Possession of a 2-year College Diploma Program or University Degree in a Human Services area.
  • Computer skills: Microsoft Word, Excel, PowerPoint, and Outlook.
  • Experience with Individual Record Management and Assessment Software.
  • Valid Ontario Driver’s License.
  • Access to a reliable vehicle
  • Automobile liability insurance coverage in the amount of at least $1,000,000.
  • Minimum 1-2 years’ experience working with individuals, families in the mental health system.
  • Professional Designation/Certification (one of the following are required):
    • Registration, in good standing, with a regulating professional body Ontario College of Social Work and Social Service Worker (e.g.OCSWSSW); or
    • Certification, in good standing, with a professional certifying organization* within health or social service fields e.g. Certified Psychosocial Rehabilitation and Recovery Practitioner (CPRRP) from Psychosocial Rehabilitation (PSR Canada);  or
    • Working towards obtaining one of the above within 6 months of hire
  • Bilingual requirements will be posted in accordance with the FLS Designation Plan. Bilingualism is required for 40% of positions. If a position is designated bilingual and no bilingual candidates (both internal and external) meet the qualifications, unilingual candidates may be considered.

SALARY INFORMATION:

  • Bachelor – Hourly, Step 1: $26.77 – Step 4: $31.49
  • Diploma – Hourly, Step 1: $24.14 – Step 4: $28.41

*Successful candidates for this position will start at Step 1 of the salary grid.
